Step 1:
Schedule your Initial Functional Medicine Consultation

This is the first stop for all new patients. Schedule your your Initial Functional Medicine Consultation to meet with Dr Zablozki-Amir. This will be by video telehealth.

You can do this directly by clicking on a Schedule a Consultation button, or by contacting our office.

Once you’ve scheduled, you’ll receive by email forms to fill out with some health questionnaires and logistical acknowledgements. You’ll also have an opportunity to upload and/or send any relevant labs and medical records for the doctor to review before your appointment.


Step 2:
Meet with Dr. Zablozki-Amir

During your Initial Functional Medicine Consultation, you and Dr Zablozki-Amir will review your medical history and set your health goals. In this in-depth appointment, she will share her opinion on your health and best next steps from a thoughtful, holistic, and practical perspective.


Step 3:
Move Forward

Based on the Initial Functional Medicine Consultation, you and Dr. Zablozki-Amir will lay out a route of how best to work further together so you can reach your health goals.

Do You Accept Insurance?

Dr. Zablozki-Amir does not bill or accept insurance directly.

Patients with out-of-network benefits may be eligible for reimbursement, and we provide a superbill at your request that can be submitted to your insurance company. Reimbursement depends on your individual plan and cannot be guaranteed.

Insurance companies determine what services, testing, and treatments they will cover and can limit flexibility in how care is structured. Remaining independent of insurance allows the practice to offer a more personalized approach without those coverage restrictions.


Are Laboratory Testing Costs Included or Covered By Insurance?

Any laboratory testing costs are additional and are customized based on your health goals and the information needed to guide your care. Lab fees are paid directly to the laboratory.

Some testing may be covered by insurance and will vary depending on the lab and your individual plan.

Dr. Zablozki-Amir does not mark up laboratory costs and will provide guidance on recommended testing and expected costs before any labs are ordered.


Why Does The Doctor Typically See Patients by Telehealth?

We see patients predominantly by telehealth. There are many benefits to this, including:

  • Convenience and flexibility: Receive care without the time and disruption of traveling to an office, making it easier to incorporate appointments into your schedule and maintain continuity of care.

  • A window into your daily environment: Connecting from your own home or workspace can provide valuable insight into the routines, habits, and factors that influence your health.

  • Greater scheduling flexibility: Telehealth allows us to offer care in a way that is more adaptable to patients’ schedules.

Visits are structured to be conducted through a secure, and simple to use HIPAA-compliant portal designed to protect your privacy and confidentiality.

While many aspects of functional medicine evaluation and care can be effectively provided through telehealth, certain situations may require an in-person assessment or additional evaluation. If this becomes necessary, we will discuss the appropriate next steps and help coordinate care with the appropriate healthcare provider.


Are there any in-person options?

Yes. In-person consultations are available in select cases. Discuss it during your Initial Functional Medicine Consultation – which will be via telehealth – to find out if it’s a good fit.

In Which States is Dr. Zablozki-Amir Licensed To See Patients?

Dr. Zablozki-Amir is licensed to practice medicine in New York, New Jersey, Connecticut, and Florida. Patients should be physically located in one of these states at the time of their telehealth visit for treatment.

If I Am Under Dr. Zablozki-Amir’s Care, Do I still Need a Primary Care Doctor?

Dr. Zablozki-Amir’s practice is designed to complement, not replace, your primary care relationship. Patients should ideally maintain an ongoing relationship with a primary care provider for urgent or acute concerns, and needs that require an in-person physical examination.


The doctor has a long name, what does she prefer to be called?

Dr. Zablozki, Dr. Ilana, or Dr. Z. Any one is fine.
